Community Care Coordinator

Comfort Keepers is looking for a dynamic person to supervise our operations at a local Independent Living Community.  This is a rewarding position for a candidate who enjoys working in a fast-paced, team-oriented environment and has a desire to improve the lives of seniors.

Our local office is growing steadily and we are looking for special individuals who have the ability to grow with us.

Responsibilities/Activities:

  • Leads direct care staff in gaining exceptional skills, dedication and compassion and serves the residents as a member of the direct care team as needed
  • Ensures that the resident’s care needs are met based on individualized needs and the plan of care
  • Informs direct care personnel of service delivery schedules
  • Communicates with clients and their family’s to ensure satisfaction and/or address concerns
  • Orients staff members to community policies and client specifics
  • Communicates new programs, interventions and new procedures with the CNA staff
  • Makes recommendations for additional training and/or staffing needs
  • Serve as primary liaison between clients, families, the community, and Comfort Keepers.
  • Represents the staff and communicates residents care needs in meetings 
  • Maintains care delivery schedules daily
  • Covers shifts with appropriate staff when unexpected changes to the schedule occur and/or  works uncovered shifts as needed when no alternatives are available
  • Assists in recruiting staff members through various means
  • Completes daily and periodic reporting
  • Regularly checks email, voice mail and/or telephone messages
  • Other duties as assigned by supervisor

Qualifications/Education

  • Senior Care Experience
  • Supervisory Experience
  • CMA or CNA preferred
  • Positive attitude, the flexibility to perform various duties in service to the residents and the ability to work in a team environment are keys to success
  • Proficient organizational, problem solving and time management skills
